Support Coordination
Community Connections Coordination and Supports are committed to assist and support you to build confidence in coordinating the necessary supports to get the most out of your NDIS plan.
We can help you with referrals that link you with tailored and individualised supports that match your needs.
We undertake all the required NDIS reporting requirements, assist you with Reassessment and Change of Circumstances and other interfacing with the agency, mainstream and community supports.

Recovery Coaching
Community Connections Coordination and Supports will walk the path with you in your psychosocial journey.
Nancy brings extensive experience with mental health and neurologcal diversity and undertakes a recovery informed approach to mental well-being. Nancy will spend time with you to support your mental health awareness and assist you to feel like you have choices and feel more in control. We can facilate you in understanding your NDIS package and support connections and collaberation with family, friends, networks and services.
We are empathetic to the fluctuations of psychosocial health.
